MHS MOBILE DENTAL is now taking applications for their annual clinics at West Wortham. Attached is the consent form you must fill out electronically for your student to participate in this program. These screenings are done voluntarily and only with consent of the legal guardian(s). Electronic forms must be complete and turned in no later than Oct. 13th. Clinic dates are tentatively scheduled for November 12th-13th and are subject to change.
