✔️ ONE form per student- FILL OUT THE FORM COMPLETELY
✔️ If you have multiple students, complete a separate form for each
✔️ Checks: For your convenience, if paying by check for multiple students, you may submit multiple forms together with one check. 
✔️ Cash: Submit to your child’s homeroom teacher along with the order form. Please try to send exact change.
